Pathways to Services Development is a California nonprofit in its build and development phase. Our work is focused on preparing a Housing First, trauma-informed model for transition-age youth in the Tri-City area and surrounding Alameda County.

Our signature model

Path to Purpose: Housing First. Transformation Always.

The Path to Purpose Initiative is our signature model. It is organized around three connected tiers: Stability Foundation, Empowerment & Healing, and Transformation & Purpose.

Our current pilot, the Rising Path TAY Program, is in structured design and development for transition-age youth ages 18–24 who are aging out of foster care or experiencing homelessness. The intended participant journey is 12–24 months, rooted in housing-first principles and long-term opportunity.

Pathways has not yet launched service delivery. This framework separates completed organizational milestones from the outcomes the Rising Path TAY Program is designed to measure once operating.

Foundation built so far

  • Governance and compliance framework

    Formal board governance and a compliance framework have been established to support responsible nonprofit development.

  • Path to Purpose Initiative designed

    The three-tier model—Stability Foundation, Empowerment & Healing, and Transformation & Purpose—has been fully designed.

  • Rising Path TAY Program in development

    The pilot has moved through structured design and development planning for transition-age youth ages 18–24.

  • Strategic funding alignment underway

    Pathways is pursuing alignment with county TAY-designated funding, HUD youth-homelessness resources, and CalAIM pathways.

How we will measure impact

  • Housing stability

    Once operating, the pilot will measure housing stability at key intervals after program entry.

  • Progress through the model

    The program is designed to track progress through Stability, Empowerment, and Transformation.

  • Education and employment advancement

    The pilot will measure participant advancement toward education, employment, or certification.

  • Continuity after program exit

    The program is designed to track continuity of support after participants exit.

We do not publish unverified participant stories. As the Rising Path TAY Program moves into service delivery, Pathways will share approved stories that honor privacy, lived experience, and community voice.
